2. Individual kitchen layout plan with scaling with equipment list and unit space planning  
3. Individual kitchen cleaning schedule
4. Declaration of Foodhandler Form 
5. Basic food hygiene course certificate of food handler 
6. Applicant's company ACRA business profile
7. Authorisation letter for filer to submit the online application on behalf of the company (if the filer is not listed as the Director in the ACRA business profile)
8. Photo of Air Cleaner, Location Plan and Pest Control Agreement
